Try following things :
Prerequistes: Install qc add-in for qtp 9.5.
answer for question 1 : Try this alternative:
i) Create a quick_test object in the test plan
ii) Click the icon to launch QTP from QC
iii) Write ur scripts and save
iv) Drag the same test object into the test lab , now u wud b able to execute the test scripts from QC.
Question 2: Do u wanna run locally or on remote host?
