Solved: 11 Years, 3 Months, 4 Weeks ago 11-24-2011, 07:31 PM
Even i faced the same issue and resolved after following the below steps;
1. Go to Start page of QTP
2. Go to Tools-->Options
3. In the general section, click on Restore Layout button.

would help others facing this same issue
