test scenarios and test cases

Test scenarios--> Test cases-->Action/Step
this is my understanding of test cases and scenarios. please correct if i am wrong.

also, i am having trouble understanding how to represent test scenarios and test cases. should both of them be different documents. if so, please provide a template.

if both the test scenario and the test case are to be in the same document, please provide da template for that too.

take the following example
1.Login is scenario
2.login with valid usename and password & login with invalid username and password are test cases.
3. navigate to login page, enter invalid username and password, click login are actions/steps
