QTP 9.5 - Unable to select an object directly from an application

Add-Ins: ActiveX, Java, VB, Web, .NET
Windows XP Pro 2002 SP2

QTP 9.5 - Unable to select an object directly from an application and add it to the object repository for use in a step. QTP fails to add the specified object – object is NOT displayed in the shared object repository tree in the Select Object for Step box after being selected via the documented “To add a test object from your application” instructions from QuickTest Professional Help page. (Please see attached for step-by-step instructions and screen shots.)

Could someone please explain why this functionality does not work?

**I've added the Object Spy information in hope it will help diagnose this problem**
