Form testing with UFT12

I installed UFT12 in my computer which is windows 7 64 bit OS. I set up the basic set up as well. I wanted to test a form in website which has 33 fields but everytime I record and play back, it says object not recognized. Also the login shows failed status. What are the steps that I should take make it run successfully? Please advise.

Looks like some thing is not set up properly. Check these tips One of those should help you.

Once add single object..in the object repository and...hightlight the object..
if it is highlighted drag the object into editor..and run.
definitely it will work..
after that add all 33 objects in similar way.if it works..
go to settings and change record and run settings..as u performed in qtp 11 and uft 11.5.
